Around the time period of [REDACTED] employee felt a pop in right shoulder resulting in pain and discomfort. Injury was mentioned to supervisor and employee requested lighter duty work options while refusing to seek medical treatment. Details of injury are uncertain as injury went further unreported for medical treatment for several months until employee sought medical treatment from doctor of choice around [REDACTED] then again in [REDACTED] when referral for MRI was suggested by medical doctor. Employee then informed employer of upcoming MRI due to a work related injury. Upon learning of this injury as work related employer referred employee to employers occupational health clinic for further medical evaluation on [REDACTED]. MRI was completed and surgical procedure to repair the shoulder injury was recommended and completed on [REDACTED].
