EE was working feeding the cardboard baler. He was sorting through a box of dunnage to separate plastic from cardboard putting plastic on the trash container and cardboard into the baler feeding band.

A forklift operator was bringing a cardboard box to the balers feeding queue as the forklift operator moved ahead he didnt realize that there was an empty pallet in the queue so he inadvertently pushed the empty pallet which pushed the cardboard box in queue which in turn pushed the TM that was working at the balers feeding point against the balers structure
