Before the incident
EE had just returned from break - [REDACTED]. EE retrieved a sit-down style PIV with clamp attachment and began loading the trailer in dock door 48. EE was not observed putting cone or wheel chocks in place nor observed installing glad-hand lock on trailer
What happened
Truck attached to trailer in dock door 48 and began to pull trailer from the dock. At the same time this occurred EE had paused the PIV at the trailer dock threshold. As the trailer pulled away the PIV fell to the ground below.
