Before the incident
After she placed a cart in position another employee removed the window punch tool from the Melzer and placed it on the cart. The window punch tool tipped over onto it 's side as it came in contact with the cart. The top piece separated and it 's this gap that her hand was in when she reached to assist in righting the the tool.
What happened
When the window punch tool was tipped upright her hand was between the top plate and the rest of the tool. The top plate closed down on her hand due to gravity.
