The employee was trained thoroughly on food safety when she was hired 2 weeks prior to the accident. At the end of each day the packing lines are cleaned. The employee was cleaning somewhere where she was not trained to be. She stuck her arm up under one of the lines and got her arm caught in the belt.

She was told in training to only clean the top of the lines. She stuck her arm up under one of the lines and got her arm caught in the belt. [REDACTED] was called and employee was taken by ambulance to the hospital. This was all turned into our workers compensation but she never contacted them after the accident. She also never returned to work or contacted us as her employer either.
