What happened
Manual disassembly of 777-wheel assembly. The automated disassembly was not functioning properly and the decision was made to manual disassemble the unit. When setting up the employee using the gun applied force prior to the employee holding the breaker bar acquiring a grip on the bar. The breaker bar spun free and hit the employee in the mouth.
