[REDACTED] was operating the AA827 camera system and was located at the exit side of the machine.
What happened
[REDACTED] was operating Fork Truck #3. [REDACTED] needed to get a gondola of parts that were stacked behind a blue container of parts. [REDACTED] lined up square with the blue container lowered forks to the ground and drove forks under the blue container. There was a stack of gondolas behind this container so [REDACTED] ensured not to allow the forks to go past the end of the blue container. [REDACTED] then slightly lifted the blue container and started to back up. He noticed the stack of containers behind the blue container moved and yelled to [REDACTED] to move. By that time it was too late and the stack of containers fell and hit [REDACTED].
