working on plasma torch

[REDACTED] and two more employees we removing frame from around the plasma torch. The two maintenance technicians removed the bolt from end cap. [REDACTED] went to the crane to move the end cap. The two other maintenance technicians went to the other end of the plasma torch to disconnect more of the frame work. As [REDACTED] was returning with the crane he was watching the chain. [REDACTED] continued to watch the chain on the crane as he moved the crane into position. He stepped behind the end cap to connect the chain. As he stepped behind the end cap it was falling. The end cap fell and hit [REDACTED] into the foot.
