What happened
Employee was bolting a combine dog house together. Another employee picked up a long part and leaned it against the doghouse in a vertical position. Employee was using an electric impact wrench to tighten the bolts. We believe the vibration from the impact wrench while tightening the bolts made the long part start to slip. The part fell hitting employee in the side of the head.
