Placing a plastic headliner into punch press machine.

While placing a headliner into punch press debris (Small Fabric - Plastics - Dusts) were not housekept. The headliner placed down into press would cause the debris to get kick up getting into the right eye of the operator. The operator had prescription glasses on and not not wearing required PPE or side shields at the time of the incident.
