Before the incident
[REDACTED] is a Registered Nurse and was working in operating room number four. At approximately [REDACTED] [REDACTED] states that he was standing up in the operating room and realized something was wrong. He stated he felt bad dizzy and asked [REDACTED] RN Circulator to replace him with another scrub tech. Ms. [REDACTED] called for a substitute tech and assisted [REDACTED] to a sitting position in a chair located in operating room number four and helped him remove his gown and other OR attire. [REDACTED] then walked with assistance from [REDACTED] to a bed located in the hall outside the OR and [REDACTED] assisted him to lay down on the bed. He was then transported to the Emergency Department on the bed by [REDACTED] RN Surgical Services Director [REDACTED] [REDACTED] and [REDACTED] for further evaluation.
What happened
[REDACTED] was not injured. He states that he recently began taking [REDACTED] about [REDACTED]. He states he did not realize [REDACTED] could cause decreased appetite and decreased thirst sensation. Upon evaluation in the Emergency Department the record indicates that [REDACTED] was severely dehydrated and was experiencing low blood pressure. He remained in the hospital on observation status (not inpatient status) until [REDACTED] [REDACTED] when he was discharged home.

No injury. This incident was a medical event caused by the employee 's dehydration and low blood pressure the result of a fairly new medication regimen.
