Installing lifting pins

When installing the lifting pins ([REDACTED]) for a medium closure head on [REDACTED] the bushing that was in one of the top bores fell out and struck the employee in the head while he was standing on the other side of the closure head. The injured employee was not yet in place to hold the bushing as the pin was coming through. The force of the nylon bushing caused a laceration on the top of his head.
