The machinist was tightening the bolts on a bevel gear with a wrench in order to affix it onto a machine.

The bit on the wrench he was using broke sending his hands to forcefully strike the gear which injured the employee with cuts on both of his thumbs. Then while waiting for first aid personnel the employee fainted and hit his head on the floor causing a laceration.

The event resulted in lacerations on the employee 's left and right thumbs (from striking the gear when the bit broke) and on the back of the head (from the fall when he fainted).
