While sewing in greige rolls [REDACTED] failed to stop the pull rollers in time. This allow the carpet to unthread. He was attempting to rethread the overhead rollers by standing on the rollers in the cradle in order to reach the carpet

He grabbed the carpet with his right hand and reached behind to reverse the J-box drive but accidentally pressed the cradle drive. This engaged the roller he was standing on pulling his left leg between two rollers.
