What happened
Employee was attempting to empty scrap hopper. In doing so employee placed left hand in pinch point between scrap hopper and PIV fork. When scrap hopper released to dump into dumpster hopper pivoted upward and pinched fingers between hopper frame and fork causing amputation to tip of left ring finger.
