Preparing material for a topping slab
What happened
Employee experienced a workplace accident while preparing material for a topping slab in [REDACTED] on the west side of [REDACTED] 16. Employee was carrying rebar on his left shoulder - moving from north to south when he encountered an elevation change of about 18 inches. This change in elevation was bridged by two stacked cinder blocks with no ladder or stairwell available to facilitate the transition. As Employee stepped onto the cinder blocks the unstable surface caused him to lose his balance and fall to his left side. In an attempt to brace himself during the fall he used his left hand which resulted in a fractured wrist.
