Before the incident
EE was at the worktable positioning himself with his left hand to hold the plate in place and used his right hand to operate the power tool moving it down the rectangular plate to back gouge the weld that was previously performed.
What happened
EE in the weld training booth was using a gouger to back gouge a weld on a 5mm plate. He had his left hand positioned holding the plate on the table and his right hand using the gouger tool. While grinding the gouger high centered and walked over the plate striking the employee 's left thumb.
