What happened
[REDACTED] Locklear was in the process of cleaning the float 2 west batch delivery chute and the bar that was being used was caught by the mixer plow arm. When the bar was hit it smashed [REDACTED] 's hands up against the metal door frame. [REDACTED] 's right hand index and middle finger were pinched along with is left hand index finger. Proper PPE was worn at time of accident. [REDACTED] was treated in house and an ambulance was called for further medical attention. The plant nurse was informed and in route to the emergency room when informed of acciden
