Before the incident
[REDACTED] was cleaning the Guering Slicer machine during product change over.
What happened
After product changeover [REDACTED] had her hand resting on onion carousel while cleaning scraps off of a front panel. There were 2 people cleaning the machine instead of just one per SOPs. The operator had communicated with the other person that production was ready to start again. The operator did not see that [REDACTED] was still cleaning with her hand resting on the carousel. Once the machine started back up the carousel took her hand and sliced open her left middle finger and her left thumb.
