What happened
[REDACTED] was pulling tubing material from a ground location and fell back into additional tubing that was in racking behind this area. He was transferred to [REDACTED] Occupational due to lower back pain. Once at [REDACTED] his breathing became very shallow and as 2 [REDACTED] nurses and myself were trying to get [REDACTED] into a wheelchair he was un-responsive and the [REDACTED] Nurses had requested me to call [REDACTED]. He was then transported to Goshen Health ER via Goshen Fire EMS services.
