WORKER WAS USING A HOSE PRESS IN A VICE TO PUSH ON FERRULES AND END COMPONENT. WHEN RELEASING THE TOOL THE DRILL BOLT WAS COMPLETELY OPEN CAUSING THE DRILL TO CATCH AND SPUN INTO WORKERS MOUTH DAMAGING TOOTH.
