What happened

EE was replacing the wheels on the forks of the pallet jack. The pallet jack slipped off the lifting jack and employee 's finger got caught under the wheel on the fork. There was a block in place as safe guard but the block was not high enough to prevent the finger from being caught between the floor and the wheel.
