What happened
The Forklift Operator drove in and began lifting the arms. At this point the arms caught the corner of Press Gate and began to shift lean and the Press Operator attempted to hold them on the forks but the weight and force of the arms carried off the forklift forks striking the Press Operator in the left ankle causing the Operator to fall to the floor.
