Before the incident
Employee was giving a SQ Heparin injection to a patient that was very thin and had very little SQ tissue who requested that she give the injection in the SQ tissue on her hip.
What happened
Employee grabbed the SQ tissue with her left hand and injected with her right hand and as she was activating the safety feature the needle came out of the skin and grazed her left 3rd finger tip.
