What happened
Reports she slipped on ice in the [REDACTED] parking lot after leaving work at approximately [REDACTED]. Reports she landed on her back with her right leg bent backwards underneath her and a car tire. Was seen in ER after incident and had x-rays done. Complains of pain in center of right knee radiating into right lower leg shin area pain worsens with movement and ambulation. All events reported per patient 's verbal account to myself at the onset of my shift (prior to her discharge from the ER) and [REDACTED] filled out by myself due to patient need to return home with infant granddaughter and daughter. Ms. [REDACTED] requests any questions to be directed to her via cell phone call.
