resident got up and when I asked him if he needed to use the bathroom he said Yes. I took him into the bathroom and after he gripped the stabilizing bars I tore his pullup sides so as to not create behaviors. He wouldn 't let go of the stabilizing bars after I asked him to. My left side of head was up by wall and out of no where he started punching my free side of head causing the left side of my head to bounce off the wall several times. I took my arms and put them around my head but he just continued to hit me over and over all the while screaming at me. My co-worker came into the bathroom and called for the nurse so they could finish toileting him. I left the room and went to the nursing desk.
