EE was walking into the building when they slipped and fell on ice The EE described ongoing pain in their lower left back left shoulder and back of their head The EE completed their shift and in followup with their supervisor on [REDACTED] indicated they were going to see their PCP the supervisor instructed to follow up with EHS prior to making personal arrangements Following review with EHS the request was made to seek follow up with the company clinic and authorization was granted on [REDACTED] The EE made the initial clinic visit after their shift on [REDACTED] and EHS received a phone call from Concentra indicating the status of the EE The EE appears to have a bruised hip and neck strain Restrictions are to include no overhead work and also a referral was made for Physical Therapy sessions to clear up the neck strain and ongoing muscle spasms
