Before the incident
IE was in the process of repairing a chain on the printer conveyor on line 8. After the repairs were made the IE left the guard off while inspecting the chain as it was running.
What happened
Once confirmed that the chain was working properly the IE attempted to stand up from the seated position directly in front of the conveyor chain and sprocket. When he stood up but shoulder came into contact with the HMI requiring him to lean forward to get out of the area. This led to his left glove coming into contact with the chain and pulling his left index finger through the sprocket.
