What happened
Associate was working with another to re-assemble a mold using the crane system. The half suspended by the crane become stuck. The injured associate after discussing how to unjam it was using the non-marring sledge hammer to dislodge the stuck half of the mold using an upswing. During one of the upswings instead of get a firm hit on the mold the hammer just glanced coming back and hitting the associate 's forehead.
