What happened
On [REDACTED] at [REDACTED] [REDACTED] was training on a reach truck for the finalization of his certification by our PIT trainer [REDACTED]. When [REDACTED] was driving from the training obstacle course in the northwest corner of the building he was conversing with another stationary training associate and continued talking while setting motion to the reach truck. [REDACTED] was driving the correct orientation on the reach truck and drove the machine toward a support beam near the training obstacle course in the northwest part of the building. Right before the collision [REDACTED] attempted to exit the reach truck before it collided with the support beam bumper guard. After the collision [REDACTED] notified management and immediately called emergency services which then [REDACTED] taken to the nearest hospital. In reaching out for information [REDACTED] was found to have a slight dislocation in is elbow and received stitches in his leg.
