What happened
Employee was working on a gear box that required some work employee removed the cover plate by hand and set it down on a pallet after working on the gear box cover plate was the last thing to go back on instead of using the eye bolts that are required to lift the plate employee took it upon himself to lift the plate by hand upon setting it down to align it the plate some how got stuck by debris (dust a piece of cloth) and would not go down the employee shifted right to left and left to right until it engaged into the holes causing the plate to go down and smashing his fingers
