On [REDACTED] at approximately [REDACTED] at [REDACTED] 's [REDACTED] I was auditing inbound trucks. I had finished my truck. I was called over to door 14 I think because the chain on the dock plate would not release when [REDACTED] tried to release it. I pulled on it a few times and it still would not release. [REDACTED] [REDACTED] and [REDACTED] were working and that dock plate and a few others. [REDACTED] acted that the chain was getting stuck on the pipe the chain goes thru. He said people were pulling it the wrong way. That 's why it was getting stuck. I felt ok but my back was a little sore. The [REDACTED] the pain was getting worse and by the evening I could not stand without going to my knees and back because of the pain. I took some pain pills I had and they did not work so I went to bed. [REDACTED] I woke up and when I stood up I had to go to the floor because of the pain.
