Employee was assigned to machine operator and noticed at approximately [REDACTED] that the conveyance chain was starting to jam due to processed product getting stuck between the rollers and belt.

Employee when to go clear the jam : as the machine was still powered on: they life the metal sheet guard to adjust the chain and resulted in the right middle finger amputation and a right ring ringer cut. [REDACTED] (witness) saw what happened and called supervisor for ER transfer.
