Climbing down a ladder.
What happened
Was previously on another machine and got a little oil on his shoe. He noticed he had oil on his shoe so we went to a mat and wiped his feet really good. A little later he went to work on a color change he was purging the machine and went down the ladder off of the platform went to step on the first step and his foot slipped and he fell straight down to the ground onto his back.
