Before the incident
Surveying area and column deciding what his tasks were going to be for the day. The employee realized the connector plate needed to be removed. Employee attempted to remove connector plate with a sledge hammer knocking the tack welds loose. After it got became loose employee put on gloves and attempted move the material back and forth to get it off.
What happened
The employee was attempting to remove a connection plate that had been tack welded onto a larger piece of material. Using a sledge hammer the employee hit the connector plate a few times to break it free. After the employee had it loose enough they put on gloves and grabbed the connector plate and began moving the material back and forth. The material broke free during an upward motion pinching both of his thumbs between the material and the connector plate injuring his left thumb.
