Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
a)   Harpers Septic Tank and Sewer Service, Ltd. employees
working at 1741 Aspen Circle, Pueblo, CO:  On or about 10/27/91 two
employees were exposed to oxygen deficient atmosphere as well as
probable exposures to hydrogen sulfide and methane gas while entering
a bar screen sewer manhole without benefit of comprehensive confined
space entry procedures in that; Elements such as, but not limited
to, the following were not addressed:
1.   A confined space entry permit system was not in place.
2.   Atmospheric testing was not done prior to confined space
entry.
3.   An Emergency response rescue program was not in place.
Among other methods, one feasible and acceptable abatement method
to correct this hazard is the implementation of a confined space entry
program in accordance with ANSI Z117.1-1989.
