Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
broken bones,  possible amputations and severe cuts from exposure to a
fall hazards or caught in moving conveyor and idler parts while
performing maintenance or lubricating the material conveyors.

Among other methods, feasible and acceptable abatement methods to
correct these hazards are:
a) Provide lockout procedures on electrical apparatus with provisions
for safe access during maintenance procedures.
b) Piping for lubricating idler bearings with provision for safe access
during maintenance procedures.
