Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or potential injury from fire and/or explosion resulting from inadequate
maintenance and inspection programs designed to eliminate ignition
sources in grain dust atmospheres:
Establishment - grain elevator, feed mill.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to develop and enforce a written maintenance and
inspection program that list critical areas for maintenance and
inspection of equipment. These areas include but are not limited to:
a) bearings;  lubrication, wear, loose bracket supports, temperature,
b) head and boot pulley; level, plumb, logging,
c) leg belt; centered on head and boot pulley, belt tension.
d) v-belts and pulleys; condition, alignment, tension.
e) elevator legs, alignment of belt, bucket bolts secured, bucket
alignment.
f) motion detection devices; function.
g) tramp metal removal devices; clean and functional.
h) dust control systems; clean and functional, lack of.
i) static electricity removal devices; functional, bonded and grounded.
j) electrical equipment; approved for hazardous location;
k) housekeeping; grain dust accumlation throughout facilities,
cleaning and frequency.
l) welding and cutting operation;  area safe before welding begins
m) fire protection; location, type, condition of fire extinguishers.
n) closing time inspection; before locking up for the night or weekend,
a complete inspection of the facility should be made by an employee
familiar with fire hazards in a mill or elevator. The inspection
should include a specific path of travel, standard items to be
inspected such as motors, drive belts, bearing, etc.

or serious physical harm to employees in that employees were exposed to
potential injury from fire and/or explosion:
Motion sensing devices were not provided in the two inside bucket
elevator legs in the feed mill.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to install motion detectors on the elevator
legs.
