Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
Employees were exposed to the hazards of being crushed, while
operating the two 1-ton, electrically operated monorail hoists, to
pick up coils of steel weighing approximately 7,680 pounds. The
overloading caused the main support posts and travel beam to deflect
in excess of 3 inches and overloaded the rated capacity of the
hoists.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
1) Immediately ensure that loads are limited to 2,000 pounds on each
hoists and 4,000 pounds on the total monorail system; and,
2) Have the hoist, hoist components, monorail, and monorail support
system subjected to a thorough examination by a competent person to
determine load capacities and to disclose any damage or wear in need
repair. A copy of the report of the person performing the examination
shall be provided to the Area Director immediately upon your receipt
thereof.
