Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free drom recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
the hazard of being crushed by the downward motion of the freight
elevator:
a) Hot End - Basement - Freight Elevator door was not equipped with
hoistway unit system door interlock. On or about 11/3/88.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to equip the freight elevator hoistway doo with
a hoistway - unit system door interlock switch so that contact of the
door to switch when door is in the open position will prevent normal
operation of freight car and hoist.  (A publication such as ANSI A17.
1-1971 may be used as a reference to determine what corrective method
should be used.)
