Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being crushed by the freight elevator due to the hoistway door being held in the open position at the first floor level. One feasible and acceptable abatement method, among others, to correct this hazard is to equip the hoistway with hoistway door interlocks in accordance with ANSI A17.1 1978 Sec. 111.3b. a) "9y Room" - Freight elevator used for transferring boxes of materials between the first and second floors.
