a) The inadvertent activation of the electric furnace in the melt
shop during maintenance operations.  Per ANSI-Z117.1-1977.
One feasible and acceptable method, among others, to correct this hazard
is to establish and enforce an adequate lock out procedure which would
include the following essentials:
1) Establish a written lock out procedure, including procedures to be
followed and training for all effected employees.
2) Periodically evaluate the program and strictly enforce all of its
provisions.
