Section 5(a)(1) of the Occupational Safety and Health Act of 1970: On %% or about 9/3/88, the employer did not furnish employment and a place of employment which were free from rocognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The inadvertent activation of the electric furnace in the melt shop during maintenance operations. Per ANSI-Z117.1-1977. One feasible and acceptable method, among others, to correct this hazard is to establish and enforce an adequate lock out procedure which would include the following essentials: 1) Establish a written lock out procedure, including procedures to be followed and training for all effected employees. 2) Periodically evaluate the program and strictly enforce all of its provisions. As stipulated in ANSI-Z117.1-1987

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: On or about 8/29/88, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Possible asphyxiation, when employees inspecting or working in the confined space of the electric melt furnace in the melt shop were exposed to chemical poisoning and or death from the hazards of oxygen deficiency and/or dangerous accumulations of toxic gases, (such as carbon monoxide) when the company failed to test the furnace atmosphere for oxygen deficiency and toxic before allowing the employees to enter. Per ANSI-Z2441.1-1982.
