Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  the
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm in that employees were exposed to the hazards
of electrical shock while wearing and operating electrical devices whose
system was not adequately grounded.
a)  Operator's Room - Directory assistant telephone
operations.  On or about 7/31/92.  Telephone
grounding
conductor was not properly connected to the
ground rod, nor was the underground telephone cable
sheath bonded or grounded.
Among other methods, one feasible and acceptable abatement to correct
this hazard is to:
1)  Provide an effective grounding path at the ground
rod as required by the National Electrical Code, 1984;
Bell Core TR-EOP-000295, Bell System Practice 802-001-191;
and Safety Rules for the Installation and Maintenance of
electric supply and communication lines, 1961.
2)  Provide effective bonding and grounding of the
underground telephone cable sheath as required by ANSI
C2-1987; and Bell Atlantic, Cable Splicers Handbook,
AT & T Practices 633-506-206, Oct. 86 and 633-506-205
