Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the Munson Mixer was not locked out to attain Zero Mechanical State (ZMS) during maintenance troubleshooting inspection, thereby exposing employees to the potential of serious bodily injury: a) Maintenance man performing inspections of the moving parts of the mixer was reaching in the path of the moving elements, on or about May 10, 1988. Method of abatement in these circumstances include but are not limited to the following: a) Establish and use lock out procedures while performing maintenance inspections on machinery.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the feed conveyor to the mixer was not locked out to attain Zero Mechanical State (ZMS) duringa manual unloading of the conveyor belt, thereby exposing employees to the potential of serious bodily injury in the event of machine activation: a) Main Shop - The mixer operator was shoveling chromite off the conveyor while standing on it, on or about May 10, 1988. Method of abatement in these circumstances include, but are not limited to the following: a) Establish and use lock out procedures while manually off loading a conveyor.
