employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees, in that: foor pedal mechanism(s)
on machinery were not protected or were inadequately protected to
prevent unintended operations from falling or moving objects or by
accidental stepping onto the pedals:
a) The foot treadle bar located on the Chicago B-3, SN: L813, mechanical
press brake, was not guarded, March 31, 1987.
b) The foot treadle (pedal) bar located on the 15 to 20 ton (estimated)
13.2 Niagara mechanical press brake located in the Press and Punch
Department, was not guarded, March 31, 1987.
c) The foot treadle bar on the 15 ton, B-1 Veson mechanical press brake
located in the Pressing and Punching Room was not guarded, March 31,
1987.
One feasible and acceptable method to correct this hazard would be:
1) Install a rigid metal guard to keep the bar or pedal from being
accidentally tripped.
*Note: Verson Brake #1 can be tripped from outside frame.
