Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
a crushing/amputation hazard during maintenance and set-up procedures
due to inadvertant stroke or activation of the sheater:
a) Hanover Plant - On the Seal Tech sheater, serial number 105728.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is the development of a Zero Mechanical policy which
includes, but not limited to:
a) Insuring that all sources of power, electrical, mechanical and
hydraulic are neutralized prior to begining maintenance.
b) Ensuring a positive means (i.e. locks) of preventing unattended
equipment start-up.
c) Ensure that the lock used is under the control of the person working
on the equipment, and the lock placed and removed by that individual.
(Note: A master key may be kept by the supervisor, but never lent.)
d) Ensure that all employees involved in these operations are trained in
the Zero Mechanical State program.
