or serious physical harm to employees in that employees were exposed to:
a) Two employees cleaning windows on the 8th floor of the Cyclops
building located at 650 Washington Road, Mt. Lebanon, PA. were
working from a boatswain's chair that was defective in that the
nylon support ropes revealed broken, cut and pulled strands
approximately 15 ft. of the parapet clamps.  Additionally, the ropes
wrapped around the thimbles were severely worn and chaffed.
