Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Two employees cleaning windows on the 8th floor of the Cyclops building located at 650 Washington Road, Mt. Lebanon, PA. were working from a boatswain's chair that was defective in that the nylon support ropes revealed broken, cut and pulled strands approximately 15 ft. of the parapet clamps. Additionally, the ropes wrapped around the thimbles were severely worn and chaffed.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical farm to emloyees in that employees were exposed to: a) Two employees cleaning windows on the 8th floor of the Cyclops building located at 650 Washington Road, Mt. Lebanon, PA. were working from a boatswain's chair that was defective in that the two metal "O" rings connect the chairs two support ropes to the parapet clamps were stretched and distorted rendering the clamps inoperative and subject to accidental opening.
