Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) Pan Room-number 2 pan. On August 19, 1988, employees were exposed to
being caught in the machinery of number 2 pan due to inadvertent
activation of the electrical system.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to initiate a formal lockout/tagout program to
ensure all electrical power circuits are positively de energized and a
formal uniform policy be established to include training and supervision
covering positive locking out of electrical circuits and the necessary
clearnaces to be given to employees.
