Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards in that:
a) On or about 8-24-89 and at times prior thereto at the White Pine Sash
Company employees were exposed to the hazard of confined space entry
and suffocation while performing the task of dislodging sawdust that
had accumulated to a height of approximately 20 feet inside the fuel
storage bin.
Feasible and acceptable abatement methods,among others, to correct this
hazard is to:
1) Implements a confined space entry program which includes as a
minimum:
Cleaning and ventilation of all tanks and vessels which contained
toxic chemicals prior to vessel entry.
Atmospheric testing for oxygen deficiency and toxic gases prior to
and during employee entry into the confined space.
Atmospheric testing for explosive vapors within the confined space
prior to and during employee entry.
Lock out and tag out of all dangerous equipment inside the confined
space.
Use of electrically operated tools and equipment and electrical
circuit interrupters where required.
Disconnecting and blind flanging of all piping entering and leaving
the confined space.
Provision of standby employee(s) constantly available at the entry
site who have suitable rescue equipment immediately available for
emergency use and who are in constant visual or voice contact with
the employees in the confined space.
Provision and use of appropriate respiratory protective equipment
during confined space entry.
Provision and use of appropriate personal protective equipment for
the head, eyes and skin during confined space entry.
Employee training on safe confined space entry procedures.
Prohibit bin entry from the bottom when sawdust is greater than 5
feet in depth.
Use of a written permit system which provides for review and
signature by responsible management officials and which includes as
a minimum the above listed safeguards.
