General-duty citation text

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) A potentially hazardous chemical (Jet-A fuel) atmosphere along with a
potential for both oxygen deficiency and explosions/fires from
combustible gases while working in confined spaces, e.g., underground
fuel storage tanks: Tank No. 3 on or about July 11, 1991, and Tank No
1 sometime in late August or early September of 1991, but before
September 18, 1991 at the "D" Concourse Tank Farm, Stapleton
International Airport, Denver, Colorado. Although a corporate
program titled, "Confined-Space Entry", and dated May 1, 1991
was available at this company location, the said written program
for confined space entry was not implemented. Therefore, the
employer did not address the following American National Standard
Safety Requirements for Confined Spaces:

Among other methods, one feasible and acceptable abatement method to
correct these hazards is to prepare and implement a written confined
space entry program in accordance with the American National Standards
Institute, "Safety Requirements for Confined Spaces" (ANSI Z117.1-1989).
Ogden Aviation Services was Previously cited for a violation of this
section of the Occupational Safety and Health Act, General Duty Clause
Section 5(a)(1) which was contained in OSHA Inspection No. 17857228,
Citation No. 1, Item No. 2, issued on September 1, 1990.
